9th Grade Students ONLY
All 9th grade students must have a current (less than 1 year) physical examination on file in the Health Office by August 1.
The CURRENT (R-01-05) form will be the ONLY form accepted.
Sports Physicals are not accepted as the 9th grade physical.
Parents must complete and sign the Health History section of the form.
The examining Physician/Nurse Practitioner is required to:
sign and date the form
include address and phone number
Students will be excluded from gym until a physical form is on file.
KEEP A COPY OF THE PHYSICAL FORM FOR YOUR FILES.
