Medical Exemption:
A current letter of Medical Exemption to Immunizations is required to be on file in the Health Office by Aug 1 or within 30 days of enrollment for Transfer-In students.
All Medical Exemptions are sent to the State of IL for review.
Religious Objection:
A current letter of Religious Objection to Immunization is required to be on file in the Health office by Aug 1 or within 30 days of enrollment for Transfer-In students.
Parents need to state their Religious Objections using their own words and listing their personal religious beliefs. Standardized forms are not needed and are discouraged.
