The 9th Grade Physical Form must list ALL of the student’s Immunizations.
The Immunization Record must include the day, the month and the year of each immunization.
The Immunization Record is required to include a minimum of:
1 current Tetanus(less than 10yrs old)
3 DPT/Td (diphtheria, pertussis, tetanus)
3 OPV (poliomyelitis))
2 MMR (mumps, measles, rubella)
3 Hepatitis B
